For over 50 years, PathWays has been providing quality services to individuals with developmental disabilities and their familiesOur region consists of Sullivan and Lower Grafton County in NH and our programs work with individuals from birth and throughout their lifespan as neededWe currently have a few Case Manager openings in a couple of our programsThe positions are located in Claremont, NH with regional travel and/or within-state travel and hybrid work may be available after your first 90 days of employmentIn addition, the positions are full-time and the starting pay range is $21.00 to $23.00 per hour depending on experiencePlease see below for the description of each:
Family Services Coordinator (Case Manager)Reports to the Director of Family Services, 2 positions availableDo you possess excellent organizational skills? Do you have at least 2 years of case management or related experience? As a Family Services Coordinator (FSC), you will assist individuals with developmental disabilities and their families by providing service coordination and advocacy while linking them to existing community and state resourcesHere are a few examples of your
Responsibilities:
You advocate and support families and individuals in a manner that increases opportunities for participation in the community by responding to the changing needs of the familyYou coordinate assessments and evaluations for all individuals as assignedYou secure all necessary referrals, documentation, and relevant data to determine the individual's unique needs including though not limited to the Support Intensity Scale (SIS) and Health Risk Screening tool (HRST)You facilitate planning meetings for individuals including completing, compiling, and processing Individual Service Agreements/Amendments/Action Plans and other related documents as requested or requiredYou participate in an ON-CALL rotation as scheduledIn other words, you serve as the hub in the wheel of an individual's and their families' support with PathWaysThis position does require travel throughout the state of NH and mileage is reimbursed.Support Broker (Case Manager)Reports to the Director of Individualized ServicesDo you enjoy assisting people reach their goals? Do you like the idea of assisting an individual or an individual in conjunction with their family to develop and manage a plan that works for them specifically? Join us as a Support Broker in our Individualized Services group and support individuals with developmental disabilitiesYou will carry a caseload of 25 to 30 individuals and coordinate phone calls, meetings, check-ins, and guidance for individuals and their familiesTo be successful in the role, you must have excellent time management skills, the ability to frequently and efficiently switch gears between tasks, and strong interpersonal skillsIt is a challenging role; however, it is always worth it to make connections with individuals and their families and to see their growth and successAfter one year, you will know you were successful if you possess a solid understanding of the Individual Service Agreement process; you have established a fluid schedule of visits and team meetings with your individuals and families; and, you have a comprehensive understanding of the needs and circumstances of each of the individuals and families you serveThis position requires regional travel with mileage reimbursed.Both positions require:
A Bachelor's degree is highly preferred; extensive Case Management experience will be considered in place of educational credentials2
years of Case Management or closely related work is requiredA minimum of 2 years of human services experience is a must with experience with developmental disabilities highly desiredExcellent verbal, written, and interpersonal communication skills are a must.Minimum of intermediate proficiency with MS Office (Word, Excel, Outlook) is required and the ability/willingness to learn new programs is a must.Willingness and ability to travel either regionally or within the state of NH are a mustAccess to reliable transportation is required.We cannot help people live their best life without you.All PathWays' positions offer a HIRING BONUSIn addition, all positions require:

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
