Full Time MSW - shared position - 20 hours at Conn Valley Homecare and Hospice and 20 hours at Associate in Medicine - Primary Care Setting (total of 40 hours)

Responsibilities:_
CVHC:
Provide and coordinate Homecare/Hospice social work services, including crisis intervention and supportive counseling to patients of all ages and their caregivers in the context of the emotional, psychological and financial concerns surrounding terminal illness. Assist patients and their caregivers in collaboration with Hospice Interdisciplinary Team and develop individual appropriate plans of care.
MSW with 1-2 years of experience in Hospice/Homecare preferred.
ASSOCIATES IN MEDICINE:
? Case Management/care coordination, particularly for individuals with chronic and/or complex medical conditions
? Patient navigation, especially for patients moving among different health care levels (e.g., inpatient, outpatient, home health, or long-term care)
? Identification and referral for specialized services, such as drug and alcohol treatment, financial counseling, and housing/utility support issues
? Education and support programming
? Assistance with medications, transportation, and advance directives
? Assessment and intervention in domestic violence situations
? Outreach and coordination with other community resources and agencies
? Community-level advocacy on behalf of patients and families
_Requirements:_
? MSW
? Current Driver's License
? 1-2 years of experience in Homecare/Hospice preferred
? 1-2 years of experience in Primary Care Setting
? Ability to communicate effectively, both verbal and written
? Assessment and evaluation experience required
? Ability to work within an EMR and have basic computer skills
? Must be able to work independently and as part of a teamEstimated Salary: $20 to $28 per hour based on qualifications.
